Afiyah grew out of Safe Life Home Health, a team that already cares for thousands of Medicare patients at home. We saw the same story again and again: older adults doing everything right, and still ending up back in the hospital. Afiyah Medical exists to change that story.

In Arabic, afiyah is the state of being completely well, body, mind, and spirit, protected from harm. It's a word people use as a blessing for the ones they love. That's the standard we hold every visit to: not just treating a condition, but protecting a whole person's wellbeing.

Afiyah is a physician-led practice. Our Medical Director is board certified in internal medicine and practices in Chicago’s western suburbs, where our patients live.
The role’s clinical focus is Afiyah’s mission in miniature: heart failure, hypertension, and diabetes, the conditions most likely to send older adults back to the hospital. The Medical Director sets our clinical protocols, oversees the Transitional Care Management program, and reviews outcomes with partner facilities every month.
